2. **Employer-Specific Documents**: Your employer must provide you with a copy of your Labour Market Impact Assessment (LMIA) and job offer letter for an employer-specific work permit, or an Offer of Employment number and job offer copy for an LMIA-exempt work permit.

4. **Proof of Payment**: You need to pay the restoration fee ($239.75), work permit processing fee ($155), open work permit holder fee ($100, if applicable), and biometrics fee ($85, if needed). Include proof of payment of all the applicable fees with your application.
5. **Explanation Letter (if applicable)**: If you lost status due to a failure to meet a condition of a permit, you must enclose a letter that fully explains why you were not in compliance.
